Scope and review status

This draft describes the forms implemented on this site. Data retention periods, service-provider agreements, tracking practices and jurisdiction-specific rights remain pending client/legal review.

Information collected

The agent fit check collects your name, email, phone number, city, state, selected agent track, licensing status and licensed states, years of experience, current organization if supplied, goals, team size, preferred contact method and time, and your explanation of interest.

The checklist form collects first and last name, email, optional mobile number, state and primary concern. Please do not submit Social Security numbers, account information, health records or other sensitive information in free-text fields.

Forms record your optional contact-consent choice, disclosure version, submission timestamp, browser user agent and IP address. The endpoint also uses a hidden spam-detection field, temporary request counters and a hashed submission identifier to reduce duplicate submissions.

How information is used

Submitted information is intended to support application review, checklist requests, responses to inquiries and requested follow-up. SMS permission is recorded separately and is not required to submit either form. The implementation only enables SMS when consent is affirmatively provided and a phone number is supplied.

Service providers and sharing

The forms submit through a Cloudflare Pages endpoint. When configured, that endpoint forwards the normalized submission and consent evidence to the designated GHL webhook. The client must confirm the receiving account, processing arrangements, access controls and any onward sharing before launch. This draft does not authorize sale of personal information or sharing of SMS consent with third parties for their own marketing.

Retention and security

Retention schedules and deletion procedures for the receiving system are pending confirmation. Temporary endpoint memory is not a durable record. No specific retention period or security certification is represented here. Access should be limited to personnel and providers who need the information for the stated purposes.

Your choices and requests

You can leave contact consent unchecked and still submit. For SMS, reply STOP to opt out or HELP for help. Contact the team below to request assistance with access, correction, deletion or contact preferences; applicable rights and procedures require legal confirmation. See the SMS Terms.

Cookies, external resources and children

The shared site layout requests fonts from Google Fonts. The homepage intro may use session storage to remember whether it has played. Any other cookies, analytics, pixels or embedded calendars must be inventoried before this policy is finalized. The forms are intended for adult inquiries; procedures for information submitted by children remain pending review.

Changes

A finalized policy should identify its effective date and explain how material changes are communicated. Those details are pending client/legal review.
